School Forms and Applications
Butler Area School District student forms and applications can be found on this page. This includes all attendance forms (including the vacation form), medical forms, enrollment forms, change request forms, athletic forms, the SHS parking application and more.
Student Handbooks

Student Attendance
Once your child enters kindergarten, school attendance is required by the Commonwealth of Pennsylvania. Visit our attendance page for multi-purpose excuse forms and other attendance related information, including how to seek attendance support for your child.
Food Service/ School Meals

Media Opt Out

Enrollment Information
Elementary Supply Lists
The supply lists for BASD grades K-5 can be found on our website. Please note we do not have supply lists for grades 6-12. Teachers will let students know if there is something specific they need for class.
Volunteer Requirements
Thank you for your interest in volunteering at the Butler Area School District. We appreciate the help and support we receive from our volunteers. Before anyone can volunteer, they must meet the volunteer requirements set forth by the Commonwealth of Pennsylvania and the Butler Area School District's volunteer policy.
